Party4Life 2014/2015

I give my persmission for my child/guardian to be shuttled by charter buses to and from KC Bowl, Koinonia Campus, Lemoore Rec Center and other venues connected with this event during the hours of 9pm on New Year’s Eve Dec. 31, 2014 until 6am on January 1, 2015.

I further release and hold harmless Koinonia Christian Fellowship and all their affiliated partners and sponsors with respect to any and all injury , disability, death or loss or damage to person or property, whether caused by negligence or otherwise.

By siging below, I understand that unruly behavior will not be tolerated at Party4Life. In the event that my son or daughter is asked to leave for non-compliance of the rules or for any other reason, I may be asked to pick him/her up. Under no circumstances will the ticket price be refunded.
